Position Overview:
We are seeking a detail-oriented and compassionate Care Planning Pharmacist to join our team. The ideal candidate will be responsible for conducting assessments, providing medication management, and supporting patients with chronic and complex health conditions through effective care planning and patient communication.
Key Responsibilities:
• Conduct Depression, Cognitive Impairment, and Medication Management assessments.
• Review patient profiles and document findings based on clinical criteria.
• Communicate with patients via outbound/inbound calls to explain care plans and answer queries.
• Develop and maintain Chronic Care Management (CCM) plans, including medications, goals, vitals, labs, and documentation.
• Coordinate appointment scheduling and billing in collaboration with providers.
Requirements:
• Bachelor’s or Doctor of Pharmacy (Pharm-D) degree.
• Prior experience in clinical or care planning roles is preferred.
• Strong understanding of chronic disease management and patient counseling.
• Excellent communication and analytical skills.
• Strong English communication skills
